The company, Heavenly Restoration Roofing Experts LLC (doing business as Heavenly Roofing), was cited for a series of willful and repeat violations during a July 2025 inspection at a Quincy job site. These violations included employees working on steep roofs without fall protection, improper ladder setup, and lack of eye protection.
The fall protection failures were classified as 'willful serious' violations, with inspectors observing employees working at heights of up to 13 feet 5 inches without any fall protection system. This violation had previously been cited in 2022 and 2023, contributing to the elevated penalties. A second willful serious violation was issued for the company's failure to provide a required written Fall Protection Work Plan, another repeat problem.
Additionally, Heavenly Roofing was cited for improper ladder setup, where side rails did not extend three feet above the landing, and for lack of eye protection, as an employee was observed using a pneumatic nail gun without safety eyewear. Both of these violations had been cited in earlier inspections.
The company was designated as a Severe Violator Enforcement Program (SVEP) case by L&I, indicating repeated or egregious safety failures. This status subjects the company to increased oversight and future follow-up inspections. The willful and repeat violations have been forwarded to federal OSHA for additional review.
Despite the reduced penalty of $52,220 after appeal, the company has not paid the fine. L&I has conducted multiple inspections over the past three years, consistently finding similar fall-protection failures. The agency has warned the company that future violations could result in additional penalties.
In response, Heavenly Roofing has taken the issues seriously, addressing concerns and making corrections. They have also implemented a payment option offered by L&I, allowing them to make payments until March 2026.
